About the Role

We are seeking a highly motivated and results-driven Clinical Navigator to join our team at PAM Health, LLC. As a Clinical Navigator, you will play a critical role in cultivating referral relationships, managing assigned territories, and completing patient assessments.

Key Responsibilities
  • Cultivate referral relationships with healthcare professionals, including physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.
  • Manage assigned territories and complete patient assessments to identify opportunities for growth and improvement.
  • Make sales contacts and analyze referral and admission data to inform business development strategies.
  • Collaborate with the business development team, including the Admissions Manager and admission staff, to facilitate referral conversion and improve customer service.
  • Develop and maintain excellent relationships with all stakeholders, including prospective patients, family members, and referral sources.
Requirements
  • Education: Degree from an accredited college or university in a health field, nursing, or a related field.
  • Current licensure in nursing or respiratory therapy is required.
  • Minimum of two years' experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
