Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Clinical Eligibility Specialist. In this role, you will be responsible for determining clinical eligibility for MassHealth recipients/applicants requiring nursing facility care.

Key Responsibilities
  • Determine clinical eligibility for MassHealth recipients/applicants for nursing facility care, adult day health, home- and community-based waiver services, and other community-based programs.
  • Complete necessary documentation requirements according to the Executive Office of Elder Affairs and MassHealth.
  • Assist in the coordination of discharge planning for elders transitioning from the nursing facility setting to the community.
  • Perform on-site evaluation and clinical assessment of consumers in nursing facilities.
  • Explore all available community alternatives with consumer, involved supports, nursing facility staff, care managers and options counselors to develop a service plan and which addresses the consumer's needs, values and preferences, with the goal to diminish barriers to receiving services in the community.
Requirements
  • A valid Massachusetts Registered Nurse license is required.
  • Applicants must have a valid driver's license and reliable transportation.
  • A Bachelor of Science in Nursing or Associate of Science in Nursing degree and at least one (1) year of clinical experience and/or nursing in a community or long-term care setting; or a Diploma RN and two (2) additional years of community-based nursing experience or long-term care direct service experience.
  • Must be willing and able to complete assessments on a regular basis and possess good written, verbal, interpersonal and computer skills and the ability to work in an interdisciplinary team-based environment.
