Job Summary:

Soliant Health is seeking a skilled pharmacist to join our team in a 13-week assignment at a reputable hospital in Kentucky. As a key member of our pharmacy team, you will be responsible for managing drug therapy protocols, reviewing medication orders, and providing education to medical staff and patients.

Responsibilities:

  • Drug Therapy Management: Collaborate with an interdisciplinary team to develop and implement drug therapy protocols, ensuring optimal patient outcomes.
  • Medication Order Review: Thoroughly review and evaluate medication orders to identify potential or actual drug-related problems, and take corrective action as needed.
  • Education and Training: Provide accurate and timely information to medical staff, healthcare professionals, and patients on medication use, side effects, and interactions.
  • Documentation and Quality Improvement: Document interventions and their outcomes to support quality improvement efforts and ensure compliance with regulatory requirements.
  • Order Accuracy and Verification: Verify the accuracy and completeness of medication orders prior to entry and after filling by technicians.
  • Pharmacy Automation and Software: Maintain up-to-date knowledge of pharmacy automated systems and computer software to ensure efficient and accurate medication management.
  • Outpatient Anticoagulation Clinic Management: Manage patients in an outpatient anticoagulation clinic, including operation of Point-of-Care testing, evaluation of results, dosing, and scheduling.
  • Additional Responsibilities: Perform other related duties as assigned by management.

Requirements:

  • Registration and Licensure: Be a registered pharmacist by the Kentucky Board of Pharmacy, active and in good standing.
  • Stress Management and Adaptability: Demonstrate the ability to work in a fast-paced environment and take appropriate action to manage stress and meet deadlines.
  • Availability and Experience: Possess full-time availability and a minimum of 1 year of recent hospital pharmacy experience or completion of a PGY-1 residency.
