Position Overview:
We are seeking a qualified Pharmacist to join our esteemed healthcare team in Kentucky. This role is essential for delivering exceptional pharmaceutical care within a hospital setting.

Key Responsibilities:

  • Collaborate with an interdisciplinary team to manage medication therapy protocols.
  • Analyze and interpret medication orders to identify and resolve potential drug-related issues.
  • Provide comprehensive drug information and education to healthcare professionals, patients, and pharmacy students.
  • Document clinical interventions and their outcomes to enhance drug therapy quality improvement initiatives.
  • Evaluate medication orders for precision and completeness prior to processing.
  • Ensure accuracy in medication dispensing through thorough checks.
  • Maintain proficiency in pharmacy automation systems and relevant software.
  • Oversee patient management in an outpatient anticoagulation clinic, including Point-of-Care testing and dosing evaluations.
  • Perform additional related duties as necessary.

Qualifications:

  • Active and in good standing as a Registered Pharmacist with the Kentucky Board of Pharmacy.
  • Ability to function effectively in a high-pressure environment.
  • Availability for full-time work.
  • A minimum of 1 year of recent experience in a hospital pharmacy setting or completion of a PGY-1 residency.
