Position Overview:
We are seeking a dedicated and experienced Hospital Pharmacist to join our esteemed healthcare team in Kentucky. This role requires a minimum of 1 year of relevant pharmacy experience.

About Soliant Health:
Soliant Health is recognized for collaborating with some of the most reputable hospital systems across the nation to provide interim pharmacy staffing solutions. We are currently looking for a pharmacist to undertake a temporary assignment at a Kentucky Medical Center.

Key Responsibilities:

  • Collaborate with an interdisciplinary team to manage drug therapy protocols.
  • Evaluate and interpret medication orders to identify and mitigate potential drug-related issues.
  • Deliver drug information and educational support to healthcare professionals, patients, and pharmacy students.
  • Document interventions and their outcomes to enhance drug therapy quality improvement initiatives.
  • Assess medication orders for precision and completeness prior to processing.
  • Verify accuracy post-filling by pharmacy technicians.
  • Maintain proficiency in pharmacy automation systems and software.
  • Oversee patient management in an outpatient anticoagulation clinic, including Point-of-Care testing and dosing evaluations.
  • Perform additional related duties as required.

Qualifications:

  • Active and in good standing as a Registered Pharmacist with the Kentucky Board of Pharmacy.
  • Ability to thrive in a high-pressure environment and respond appropriately.
  • Full-time availability is essential.
  • At least 1 year of recent hospital pharmacy experience or completion of a PGY-1 residency is required.
