Soliant Health is seeking a highly skilled and experienced pharmacist to join our team in a hospital setting in Kentucky. As a pharmacist with Soliant, you will have the opportunity to work with a reputable healthcare organization and contribute to the delivery of high-quality patient care.
Key Responsibilities- Manage drug therapy protocols as part of an interdisciplinary team to ensure optimal patient outcomes.
- Review, interpret, and evaluate medication orders to identify, prevent, and resolve potential or actual drug-related problems.
- Provide drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students to promote safe and effective medication use.
- Document interventions and their outcomes for use in drug therapy quality improvement efforts to enhance patient care.
- Evaluate medication orders for accuracy and completeness prior to order entry and check for accuracy after filling by technicians to ensure medication safety.
- Maintain adequate knowledge of pharmacy automated systems and computer software to optimize workflow and patient care.
- Ability to manage patients in an outpatient anticoagulation clinic, including operation of Point-of-Care testing, evaluation of results, dosing, and scheduling.
- Perform other related duties as assigned to support the pharmacy team and contribute to the success of Soliant Health.
- Registered Pharmacist by the Kentucky Board of Pharmacy, active and in good standing.
- Ability to work in a stressful environment and take appropriate action to ensure patient safety and well-being.
- Full-time availability to meet the needs of the hospital and patients.
- Minimum 1 year of recent hospital pharmacy experience or completion of PGY-1 residency to ensure relevant skills and knowledge.
