Home Health Care Nurse

Job Summary:
We are seeking a compassionate Registered Nurse to join our team in providing high-quality care to patients in their homes. As a Registered Nurse, you will be responsible for creating and managing personalized care plans, administering medications, and monitoring patient conditions.
Responsibilities:
- Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications:
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver's license
- First aid/CPR certification required
