Nurse Care Specialist

Job Summary
We are seeking a skilled Nurse Care Specialist to join our team at Symbii Home Health and Hospice. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to individuals in their homes.
Key Responsibilities
Conduct initial assessments of patients and their families to determine home care needs
Develop and implement comprehensive care plans that meet the unique needs of each patient
Provide hands-on nursing care, including administering medications and treatments as prescribed by physicians
Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
Collaborate with community health professionals to coordinate care plans and ensure optimal patient outcomes
Requirements
RN licensed in the State of Idaho
Valid Driver's License
Home Health and Hospice experience preferred
