Job Summary:

We are seeking a skilled Registered Nurse to provide exceptional care to patients in the Hospice and Home Health setting. As a Weekend On-Call RN, you will be responsible for creating and managing personalized care plans, coordinating with colleagues, and administering medications.

Responsibilities:

  • Assess, plan, implement, and evaluate patient care plans
  • Administer medications and monitor patients for side effects or reactions
  • Document any changes in patient conditions and report on changes
  • Maintain accurate records of patient care and discuss observations with the supervisor
  • Rotating weekend on-call schedule

Qualifications:

  • Currently licensed Registered Nurse (RN) in state of employment
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver's license
  • First aid/CPR certification required
