Job Title: Case Manager

We are seeking a skilled and compassionate Case Manager to join our team at High Point Treatment Center. As a Case Manager, you will play a vital role in supporting individuals and families in achieving their recovery goals.

Job Summary:

The Case Manager will work closely with residents, program managers, and other stakeholders to provide individualized support and services. This includes facilitating intakes, developing and implementing individual service plans, and connecting residents with community resources.

Responsibilities:
  • Maintain a safe and secure environment for residents and staff
  • Ensure timely contact with program managers and authorities in crisis situations
  • Understand and follow established referral procedures and schedule intakes
  • Facilitate intakes of new residents, including setting up new records and completing intake paperwork
  • Develop and implement individual service plans with residents
  • Referrals for needed services to appropriate agencies/providers
  • Maintain accurate case files and ensure required authorizations are signed by residents
  • Prepare legible case management notes of each session and document referrals on designated forms
  • Update individual service plans monthly and ensure periodic evaluations are done timely
  • Accurately record resident attendance at meetings, appointments, and groups
  • Meet with each resident bi-weekly to review and evaluate progress in treatment and recovery
  • Attend monthly case review with program managers
  • Attend clinical supervision, required staff meetings, and trainings
  • Ensure timely completion of admission/discharge satisfaction surveys by residents
  • House driving/transportation
  • Ability to work with underserved and disenfranchised families
  • Comfortable working in diverse environments and using translation and interpretation tools
  • Transport clients
  • All other duties as assigned by the program manager
Requirements:
  • High School Diploma or GED and one year of experience in the substance use treatment field preferred
