We are seeking a highly motivated and compassionate Therapeutic Mentor to join our Community Based Services team. As a Therapeutic Mentor, you will work with youth and families who require behavior management to address mental health needs. Your primary responsibility will be to provide a variety of community-based services, including skill coaching, job coaching, life skills training, activity planning, community exploration & integration, behavior management, assessment, treatment planning services, crisis management, and case management services.
Key Responsibilities:- Provide community-based services to youth and families, including skill coaching, job coaching, and life skills training.
- Develop and implement individualized treatment plans to address the unique needs of each youth and family.
- Collaborate with other members of the treatment team to ensure comprehensive and coordinated care.
- Provide crisis management and case management services as needed.
- Maintain accurate and timely documentation of services provided.
- Bachelor's degree in a human service field from an accredited university.
- One year of direct relevant experience working with youth and families who require behavior management to address mental health needs.
- Valid driver's license and reliable transportation.
- Ability to obtain and interpret information in terms of the needs of emotionally fragile, mentally ill youth and their families.
