About the Role

As a Healthcare Navigator, you will be responsible for facilitating the patient journey, ensuring seamless navigation throughout the healthcare system upon discharge. You will work closely with patients, families, and healthcare teams to coordinate care, identify resources, and provide support.

Key Responsibilities
  • Coordinate patient care transitions, ensuring timely and effective transfer between healthcare settings.
  • Identify and connect patients with relevant community resources, services, and providers.
  • Collaborate with healthcare teams to develop and implement personalized care plans.
  • Provide education and support to patients and families, empowering them to make informed decisions about their care.
  • Maintain accurate records and communicate effectively with patients, families, and healthcare teams.
Requirements and Qualifications
  • Bachelor's degree in Nursing or related field.
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Experience working in healthcare, preferably in a navigator or similar role.
  • Excellent communication, interpersonal, and problem-solving skills.
