The successful candidate will work closely with interdisciplinary teams to identify patients' needs, develop personalized care plans, and coordinate services to ensure optimal outcomes.

Responsibilities
  • Provide expert clinical guidance to patients, families, and caregivers to navigate the healthcare system effectively.
  • Conduct thorough assessments of patients' needs, identifying areas for improvement and developing strategies to address them.
  • Collaborate with healthcare professionals to develop and implement comprehensive care plans that meet individual patient needs.
  • Coordinate referrals to specialist services, social services, and other support resources as needed.
  • Develop and maintain relationships with community-based services and organizations to enhance access to care.
Requirements
  • Bachelor's degree in Nursing or related field; Master's degree preferred.
  • Current licensure as a Registered Nurse in the state of employment.
  • A minimum of 2 years' experience in acute care nursing, preferably in emergency medicine or critical care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
  • Ability to work independently and collaboratively as part of a team.
