Patient Care Manager

As a Patient Care Manager at Sterling St James LLC, you will play a crucial role in overseeing the delivery of care to our patients. Your key responsibilities will include:
Core Responsibilities:
1. Facilitating effective communication and collaboration regarding care requirements as reported by case managers.
2. Maintaining and updating patient schedules to ensure seamless and high-quality care delivery.
3. Organizing case conferences and diligently following up on assigned action items.
4. Providing guidance and mentorship to clinical staff and support roles.
5. Addressing team concerns and fostering professional development among team members.
6. Supervising and directing the activities of team personnel.
7. Managing operations in the absence of senior management.
8. Overseeing clinical, administrative, and documentation processes.
9. Responding to inquiries with empathy and professionalism.
10. Participating in an on-call nursing rotation and delivering skilled nursing services in patients' residences.
Qualifications:
To be considered for this role, candidates must meet the following criteria:
- Current licensure as a registered nurse.
- Valid CPR certification.
- Previous experience in home care and supervisory roles is preferred.
- Demonstrated ability to work effectively in a multidisciplinary environment.
- Must possess a valid driver's license and have access to a reliable, insured vehicle.
- Familiarity with professional standards and regulations.
- Excellent observational, nursing judgment, and communication skills are essential.
