Key Responsibilities:
- Assessment and Care Planning: Conduct thorough assessments of patients' physical, emotional, and social needs, and develop comprehensive care plans that address their unique requirements.
- Interdisciplinary Collaboration: Work closely with attending physicians, nurses, social workers, and other healthcare professionals to ensure coordinated care and optimal patient outcomes.
- Communication and Documentation: Maintain accurate and timely records of patient interactions, progress notes, and care plans, and communicate effectively with patients, families, and healthcare teams.
- Case Management: Manage patient caseloads, including admission processing, eligibility determination, and ongoing care coordination.
