We are seeking a skilled Targeted Case Manager to join our team. As a Targeted Case Manager, you will play a critical role in providing blended case management services to adults 18 and over with a serious mental health diagnosis.

Key Responsibilities:

  • Assess clients' needs and develop service plans in conjunction with the client.
  • Link clients to community resources and services to promote independence and stability.
  • Monitor clients' progress and adjust service plans as needed.
  • Collaborate with healthcare providers and community agencies to ensure comprehensive care.
  • Provide crisis management and verbal de-escalation support to clients.
  • Establish treatment programs and coordinate services to meet clients' needs.
  • Verify clients' attendance at providers and observe and evaluate treatments and responses.
  • Maintain accurate records and submit documentation for all services provided.

Requirements:

  • Fluent in English and Spanish preferred.
  • BA + 2 years CMI experience (preferred degree in Social Work, Psychology, or other Human Service field).
  • Valid driver's license required.
  • Team approach and TQM philosophy a plus.
