Job Summary

As a Case Manager with The Kintock Group, you will play a critical role in providing intake, assessment, and case management services to our clients. Your primary responsibility will be to develop and implement comprehensive care plans that meet the unique needs of each client.

Key Responsibilities
  • Conduct thorough assessments of client needs and develop individualized care plans
  • Provide clients with information on community services and agency resources
  • Educate clients on program participation, including rights and responsibilities
  • Utilize case management skills to link clients to resources and services
  • Collaborate with clients to develop and revise care plans
  • Participate in case conferences to discuss client progress and develop care plans
  • Ensure all services are supported by care plans and documented
  • Transport clients as needed
Requirements
  • Excellent communication and interpersonal skills
  • Minimum six-months of relevant experience
  • Valid driver's license
  • Bi-lingual (English/Spanish) preferred

Knowledge of Microsoft Office Suite and ACA requirements is a plus.
