Job Summary

We are seeking a skilled and compassionate Clinical Admissions Nurse to join our team. As a Clinical Admissions Nurse, you will be responsible for ensuring that our clients receive the highest level of care and support. This will involve assessing client needs, developing and implementing care plans, and providing education and support to clients and their families.

Key Responsibilities
  • Client Care: Assess client needs, develop and implement care plans, and provide education and support to clients and their families.
  • Supervision and Leadership: Supervise and mentor staff, provide guidance and support, and ensure that all staff members are meeting the highest standards of care.
  • Education and Teaching: Provide education and training to clients and their families, and stay up-to-date with the latest developments in home care.
  • Financial: Ensure that all clients' authorization for services is current and that the quantity and type of services provided meet payer requirements.
  • Community Relations: Develop and maintain relationships with referral sources, including physicians, hospitals, and community resources.
Requirements
  • Education: Currently licensed as a R.N. in the state of Minnesota.
  • Experience: At least two (2) years experience in a health care facility or home care field.
  • Skills: Strong organizational skills, ability to work independently, and excellent written and oral communication skills.
  • Certifications: Current CPR certification recommended.
