Job Title: Outreach Case Manager

PATH is seeking a highly motivated and compassionate Outreach Case Manager to join our team in West Los Angeles. As a key member of our Outreach Team, you will play a critical role in helping individuals experiencing homelessness achieve stability and independence.

About the Role

The Outreach Case Manager will work closely with clients to assess their needs, develop individualized service plans, and provide referrals to resources that support their goals. This position requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities
  • Conduct intake and individualized needs assessments for clients
  • Develop and implement individualized service plans to address clients' housing goals, income, and other personal needs
  • Provide referrals and linkages to resources that support clients' goals
  • Evaluate clients' strengths and challenges to inform service planning
  • Maintain accurate and up-to-date client files and documentation
  • Collaborate with other team members to ensure seamless service delivery
Requirements
  • 2+ years of experience in healthcare, case management, or a related field
  • Strong analytical and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Valid driver's license and reliable transportation
