Job Summary

PATH is seeking a highly motivated and compassionate Outreach Case Manager to join our team. As a key member of our Outreach Team, you will play a vital role in connecting individuals experiencing homelessness with essential services and resources. Your dedication and expertise will help individuals break the cycle of homelessness and achieve their goals.

Key Responsibilities
  • Conduct intake and individualized needs assessments for clients, developing personalized service plans to address housing goals, income, and other personal objectives.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their service plan goals.
  • Evaluate strengths and challenges to address short-term and long-term goals, conducting 90-day reviews with clients after initial intake.
  • Maintain accurate and up-to-date client files, adhering to PATH policies and documentation standards.
  • Develop therapeutic and professional relationships with clients and systems/partners to enhance efforts to engage and retain individuals with their housing goals.
Requirements
  • Two years of experience in healthcare, case management, homeless services, or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Valid driver's license and reliable transportation.
