Outreach Case Manager

We are seeking a highly skilled and compassionate Outreach Case Manager to join our team at Special Service for Groups, Inc- HOPICS. As a key member of our multidisciplinary team, you will play a vital role in providing professional case management services to homeless individuals and families in Service Planning Area 6.
Key Responsibilities- Conduct comprehensive needs assessments with adult consumers to identify and address immediate and long-term needs.
- Develop and implement individualized treatment plans in collaboration with the multidisciplinary team.
- Provide case management services, including linkages to benefits, housing supports, mental health, physical health, and substance use programs.
- Work closely with clients to resolve or cope with issues contributing to homelessness, including mental, emotional, medical, and substance-related problems.
- Document all interactions and treatment plans in accordance with established guidelines and regulations.
- Collaborate with community organizations, law enforcement, and other stakeholders to provide comprehensive services to homeless individuals and families.
- Participate in regular supervision, training, and meetings to enhance skills and knowledge.
- Some credits from an accredited college or university, or at least two years of experience working with homeless individuals.
- Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
- Ability to communicate effectively both written and orally with collaborative entities.
- Neat and professional appearance.
- Attention to detail and punctuality are required.
- Working knowledge of Microsoft Word, Excel, and Internet.
- Experience working with homeless persons with co-occurring issues (mental/health/disabilities/substances).
- Ability to work with co-occurring populations that present with any stand-alone or combination of mental illness, health conditions, and substance-related concerns.
This position requires working in various environments, including homeless encampments, and interacting with homeless individuals, individuals with severe physical and mental disabilities, and individuals with infectious diseases. The incumbent must be able to work effectively and produce quality work under pressure, while dealing with unpredictable situations, crises, and constant distractions.
Physical and Mental RequirementsThe incumbent typically spends time sitting, standing, typing, thinking, writing, walking, carrying (max. 25 lbs.), listening, speaking. The incumbent must be able to perform effectively under any and all of the following: unpredictable situations, crises, constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think, and conceptualize.
