Case Manager I

St. Joseph Center is seeking a highly motivated and compassionate Case Manager I to join our team. As a Case Manager I, you will play a critical role in providing holistic and coordinated outreach and support to individuals experiencing homelessness.
Job SummaryThe Case Manager I will work as part of a multidisciplinary team to engage with individuals living in recreational vehicles and automobiles, with the goal of moving them off the streets and into permanent housing. You will be responsible for developing and maintaining strong ties to the community, law enforcement, and other homeless service providers to support the programmatic goals of the Vehicular Homelessness Outreach Program (VHOP).
Key Responsibilities- Coordinate with relevant departments and community partners to provide outreach and support to individuals experiencing homelessness.
- Develop and maintain strong relationships with community stakeholders, including law enforcement and other homeless service providers.
- Organize and participate in case conferencing meetings to develop case plans and prioritize workloads.
- Provide programmatic reporting and periodic updates to senior staff.
- Three years' experience providing homeless outreach, case management, and/or other related services to highly vulnerable populations.
- Bachelor's degree from an accredited college or university in Psychology, Social Work, Human Services, or a related field.
- Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed.
St. Joseph Center offers a competitive salary range of $22-$24/hour, as well as a comprehensive benefits package and opportunities for professional growth and development.
