Job Summary

Special Service for Groups, Inc. is seeking a highly skilled and compassionate Lead Outreach Case Manager to join our HOPICS team. As a key member of our multidisciplinary team, you will be responsible for providing professional case management services to homeless consumers, linking them with essential benefits, housing supports, and other mental health, physical health, and substance use programs.

Key Responsibilities
  • Conduct comprehensive needs assessments with adult consumers to develop effective treatment plans
  • Collaborate with internal and external entities to provide holistic support services
  • Establish and maintain relationships with community organizations and stakeholders to enhance service delivery
  • Document all interactions and interventions in accordance with established standards of care
  • Participate in regular supervision and training sessions to enhance skills and knowledge
  • Assist with data entry and reporting requirements, including HMIS and LAHSA reports
  • Represent the agency and division in a professional manner at meetings and community events

Requirements
  • Some college credits or at least two years of experience working with homeless individuals
  • Ability to work with diverse populations, including those with co-occurring mental health and substance use issues
  • Excellent communication and interpersonal skills
  • Working knowledge of Microsoft Office and HMIS
  • Valid driver's license and auto insurance
  • Ability to work in unpredictable situations and crises

Working Conditions
This position requires working in various environments, including homeless encampments, and interacting with individuals with severe physical and mental disabilities. The incumbent must be able to work effectively under pressure and maintain a professional demeanor in challenging situations.
