Position Overview


Our Primary Care-Senior Services team in the Austin/Albert Lea area is looking for an Advanced Practice Provider to join a multidisciplinary care team. This role involves delivering care to community members through our Senior Services Skilled Nursing Facility (SNF) practice.

This position offers a high degree of autonomy, while also allowing for collaboration with internal medicine physicians, registered nurses, and other healthcare professionals to ensure comprehensive care.

The scope of care includes, but is not limited to, acute, chronic, preventive, and palliative services for patients in SNFs (post-acute and long-term care settings) within the SEMN MCHS region, primarily located in Austin/Albert Lea.

Responsibilities encompass comprehensive evaluations, ordering and interpreting diagnostic tests, prescribing medications and treatments, and providing patient counseling.

Qualifications


For Nurse Practitioners:
Preferred candidates will hold a Master's or Doctoral degree from an accredited nursing and Nurse Practitioner program. A current Minnesota RN license and certification as an ACNP, ANP, GNP, or FNP is required.


For Physician Assistants:

Applicants must be graduates of an accredited Physician Assistant program, possess current PA certification from NCCPA, and be registered as a PA with the Minnesota State Board of Medical Practice. Eligibility for DEA registration is also necessary.

Additional Qualifications

While a minimum of one year of experience in a medical clinic or hospital setting is preferred, strong written and verbal communication skills are essential. Candidates should demonstrate the ability to work collaboratively with all members of the healthcare team. Experience in geriatrics, primary care, or internal medicine is advantageous.
