Job Summary

Catholic Charities of the Archdiocese of Chicago is seeking a skilled Case Manager to join our team in the Veterans Housing Program. As a Case Manager, you will play a critical role in providing supportive services to clients and their families, promoting self-sufficiency and overall well-being.

Key Responsibilities
  • Develop and implement individualized service plans to meet the unique needs of clients
  • Provide case management and supportive services to clients, including advocacy, counseling, and referrals to community resources
  • Conduct regular home visits and assessments to evaluate client progress and identify areas for improvement
  • Maintain accurate and up-to-date client records and databases
  • Collaborate with community partners and stakeholders to ensure seamless service delivery
  • Participate in community outreach efforts to promote the program and services
  • Meet with clients as required by the program or based on client needs
Requirements
  • Master's Degree in Social Work or related field
  • 3+ years of experience in case management or a related field
  • Strong knowledge of community resources and services
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
Working Conditions

The Case Manager will work in a fast-paced, dynamic environment, interacting with clients, community partners, and internal stakeholders. The position requires strong organizational and time management skills, as well as the ability to work independently and as part of a team.

Physical Demands
  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day
  • Push and pull objects up to 25 pounds
  • Climb up and down up to 6 flights of stairs at a time
  • Lift up to 25 pounds

  • Agency-specified automobile insurance
Supervision

The Case Manager will report to the Program Director and will not have direct supervision responsibilities.
