Veterans Support Specialist

Overview:
The Veterans Support Specialist is an integral part of the Health Care for Homeless Veterans (HCHV) Program within the Homeless Department in Mental Health. This role focuses on delivering essential outreach, assessments, referrals, and case management services tailored for homeless veterans within the community.
Key Responsibilities:
The incumbent will be tasked with the following duties:
1. Conduct comprehensive psychosocial evaluations to identify the needs of veterans and their families.
2. Actively engage in the collaborative treatment planning process alongside multidisciplinary teams.
3. Participate in discharge planning efforts, ensuring timely and appropriate execution of discharge strategies.
4. Provide educational resources and consultations to veterans and their families regarding community services, advance directives, and VA benefits.
5. Serve as a liaison between the VA and community resources, including marketing and public relations responsibilities as necessary.
6. Deliver case management support to veterans and their families throughout their care continuum.
7. Offer a complete range of social work services adhering to established standards, including casework, discharge planning, and additional relevant services.
8. Address Advance Directives and organ donation matters in line with the operational standards of the work area.
9. Facilitate wellness and prevention education and lead patient/family support groups as appropriate.
10. Coordinate services, maintain follow-up, and communicate effectively with care lines to ensure continuity of care for veterans.
Work Schedule:
Monday to Friday. This position is not virtual.
Eligibility and Requirements:
To qualify for this position, candidates must be U.S. citizens and meet specific educational and licensure requirements. A master's degree in social work from an accredited institution is mandatory, along with state licensure to practice independently at the master's level. Candidates may be considered based on pending educational or licensure requirements, provided they fulfill all necessary criteria before hiring.
Professional Development:
This position is eligible for the Education Debt Reduction Program (EDRP), offering student loan repayment assistance based on individual eligibility criteria.
Important Notes:
Participation in vaccination programs for seasonal influenza and COVID-19 is a requirement for all personnel in this role.
