As a Registered Nurse Care Coordinator, you will:

This role may be ideal for you if you are committed to prioritizing patient-centered care. Here’s what you will be responsible for:

  • Managing referrals and ensuring timely assignments of clinicians and therapists for patient evaluations.
  • Coordinating the assessment of patient home health benefits and medical necessity.
  • Continuously evaluating patient needs and ensuring that care is tailored to individual requirements, aligns with home health eligibility criteria, and adheres to physician directives.
  • Overseeing the creation, implementation, and regular updates of personalized patient care plans.

Qualifications:

  • Valid RN licensure in the state of Texas.
  • Current CPR certification is mandatory.
  • Possession of a valid Driver's License, vehicle insurance, and access to a reliable vehicle.
  • Preferred: At least one year of professional nursing experience in home health, with familiarity in OASIS.
