Job Summary

We are seeking a highly skilled and compassionate Care Coordinator RN to join our team at the Center for Health Care Services. As a key member of our Integrated Care team, you will play a vital role in ensuring that our patients receive comprehensive and coordinated care.

Key Responsibilities
  • Coordinate services between the Integrated Care team and community services to ensure that patients receive holistic care.
  • Establish and maintain collaborative relationships with healthcare providers, community organizations, and patients to achieve benchmark standards and improve patient outcomes.
  • Provide education and support to patients on health issues, community resources, insurance information, and financial assistance.
  • Participate in Quality Improvement activities, including data collection, tracking, and analysis.
  • Supervise the work of staff and interns, providing feedback and performance evaluations.
  • Provide professional nursing care to patients, including assessments, nursing diagnoses, and care planning.
  • Maintain accurate records and complete reports.
  • Develop and coordinate wellness programming, including promotion of activities, tracking attendance, and evaluations.
  • Participate in Quality Assurance and Quality Improvement activities.
Requirements
  • Current Registered Nurse Certification to practice in the State of Texas.
  • Two (2) years of nursing experience.
  • Current Registered Nurse Certification or LPC, LMSW license.
  • Valid driver's license and automobile insurance coverage.
  • Ability to meet physical requirements of the job, including lifting up to 12 lbs. and supporting up to 55 lbs.
Preferred Qualifications
  • Bachelor's degree or higher in Nursing from an accredited school of nursing.
  • Previous case management experience.
  • Bilingual (English / Spanish).
  • Experience with or knowledge of wellness-oriented trauma-informed care.
  • Previous lead or supervisory experience.
Competencies for Success
  • Knowledge of applicable software applications, care coordination plans, and federal, state, and local laws, rules, or regulations in area of nursing practice.
  • Skill in mediation and counseling, and ability to maintain confidentiality and professional boundaries.
