CareFor is actively seeking a dedicated RN with a Bachelor's degree to become a vital part of our Care Management team in a full-time role.

The RN Care Coordinator is responsible for crafting, executing, and assessing personalized care strategies, advocating for patients, and acting as a bridge between patients, their families, and various healthcare professionals to fulfill individual needs in home and community contexts.

- Conduct initial assessments to identify patient needs, including medical, social, familial, and living conditions, with ongoing reassessments as necessary.
- Perform health evaluations to ascertain nursing requirements.
- Design, implement, and review a continuous care plan tailored to individual patient needs.
- Make home visits based on patient requirements, adjusting visit frequency as necessary.
- Inform physicians of any changes in patient conditions.
- Coordinate care with other healthcare or community providers, including home health, hospice, nursing facilities, physicians, therapists, or hospitals to ensure comprehensive patient support.
- Accompany patients to medical appointments as needed to advocate for their healthcare needs.
- Participate in on-call rotations.
- Must be a registered nurse (RN) licensed in Texas or comply with the Texas Board of Nursing regulations for Nurse Licensure Compact (NLC).
- A minimum of one year of clinical experience as an RN is required.
- Strong written and verbal communication skills are essential.
- Proficiency in reading, writing, and understanding English is necessary.
- Comfortable navigating Electronic Medical Records and possessing basic computer skills.
- Ability to triage patient needs and prioritize daily tasks, adapting to changing circumstances.
- Critical thinking skills to creatively solve problems are required.
- Physical capability to lift, bend, and squat as determined by job responsibilities.
- Must provide proof of a valid driver's license and have reliable automobile insurance.
- Professional appearance is expected.
- Must be at least 18 years of age.
