Position Overview:

This role is essential for managing care to evaluate and coordinate comprehensive medical and supportive services for complex populations with significant health needs. The objective is to foster quality and cost-effective care through personalized service plans.

  • Assessment and Planning: Analyze the service requirements of high-risk members and propose effective care strategies.
  • Continuous Evaluation: Develop and regularly update long-term care plans, collaborating with the care management team to identify necessary providers and resources.
  • Coordination of Care: Facilitate communication between members, families, and care providers to ensure optimal person-centered services.
  • Monitoring Progress: Track care plans and member conditions, adjusting strategies as needed to meet individual goals.
  • Data Review: Examine member information to identify trends and enhance care quality in compliance with regulations.
  • Education and Advocacy: Provide guidance to members and families regarding healthcare options, resources, and benefits.
  • Community Resource Coordination: Assist members in accessing community services and support systems.
  • Leadership Collaboration: Work with management to improve care quality and service delivery efficiently.
  • Training and Support: Mentor new hires, ensuring they are equipped for success in a Managed Care Organization.

Qualifications:

Education: Graduate from an accredited nursing program or a Bachelor's degree with relevant experience. A Bachelor's degree in Nursing is preferred.

Licensure:

  • Current RN licensure in the state of Texas or a compact state.
  • Current Nurse Practitioner licensure as applicable.
  • Resource Utilization Group (RUG) certification is required for superior candidates.

Compensation: The pay range is $62.54 per hour.
