Position Overview:

Role Summary:

The Healthy Communities initiative is a vital program aimed at fostering unity within at-risk populations while enhancing access to healthcare and support services through outreach and education. This initiative also focuses on identifying the specific needs of diverse communities and formulating strategic plans to meet those needs. The service provided will directly tackle public health challenges in these communities, ensuring that healthcare and support services are readily available through adaptable service delivery methods. Members will engage in community outreach, educational activities, patient involvement, and facilitate connections to healthcare services and community resources. Additionally, members will take part in local and statewide initiatives that raise awareness about health and available services.

This role of Precision Medicine Community Advocate is integral to the All of Us Research Program (AoURP) across various Community Health Center locations, primarily focusing on the Meriden site. The AoURP is a national initiative funded by the NIH, aiming to enroll a diverse population in a long-term study that examines how individual differences in genetics, environment, and lifestyle influence health outcomes. The objective is to provide researchers with insights that could lead to more personalized healthcare solutions. The Community Health Center, Inc. has been selected to assist in enrolling patients into this significant program.

Benefits and Terms:
  • One-year service commitment with a minimum of 32 hours per week, totaling 1700 hours.
  • A stipend of $22,000 distributed bi-weekly throughout the service term.
  • An educational award of $7,395 upon successful completion of the service term.

Key Responsibilities:
  • Facilitate monthly health education workshops for patients, ensuring completion of pre- and post-surveys and managing data entry for these sessions.
  • Document and track all patient engagement activities within electronic health records.
  • Design and implement educational sessions, such as lunch-and-learn events, focusing on precision medicine and the importance of the All of Us Research Program.
  • Create engaging social activities for participants, including arts and crafts related to precision medicine.
  • Regularly recruit and invite participants for scheduled AoURP events.
  • Research and develop content for educational sessions and social activities.
  • Produce promotional materials for precision medicine and patient education, both digitally and in print.
  • Promote and coordinate educational sessions and social activities, ensuring successful facilitation.
  • Engage in community outreach and represent the program at various events.
  • Deliver presentations to community partners about precision medicine and the All of Us Research Program.
  • Collaborate with community organizations to educate local populations on precision medicine and co-host events.
  • Participate in regular team meetings to share updates on engagement strategies and event outcomes.
Qualifications:
  • High school diploma or equivalent required.
  • Proficient in Microsoft Office and internet applications.
  • Strong time management and organizational capabilities.
  • Excellent verbal and written communication skills.
  • Ability to problem-solve and maintain composure in challenging situations.
  • Successful completion of all required background checks.
  • Willingness to travel between various sites as needed.
  • Availability to serve during evenings or weekends for community events and educational sessions.
Preferred Qualifications:
  • Associate's degree in public health or a related field.
  • Experience in healthcare settings.
