Care Management Associate Job Description
**About the Role**
As a Care Management Associate at Norton Healthcare, you will play a vital role in facilitating timely and safe transitions of care for our patients. You will work closely with our multidisciplinary care team to ensure seamless coordination of post-acute services.
**Responsibilities**
* Collaborate with the care management team to identify patient needs and develop effective care plans
* Work closely with medical and nursing staff to coordinate post-acute services
* Provide patient advocacy and support to ensure timely and safe transitions of care
**Qualifications**
* No experience required; comprehensive on-the-job training provided
* One year of clinical experience in a healthcare setting desired
* High School Diploma or GED required
* Bachelor's Degree preferred
