Hospice Nurse Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Home Care team as a Hospice Nurse. The successful candidate will provide high-quality patient care, coordinate services, and work collaboratively with the interdisciplinary team to ensure the best possible outcomes for our patients and their families.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans that address physical, emotional, and spiritual needs
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care
  • Collaborate with the interdisciplinary team to coordinate services and ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Current RN licensure in the state of Arkansas
  • Minimum one year of hospice experience preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
