Our Hospice Home Care team is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care and support to our patients and their families.

**Job Summary:**
As a Hospice LPN, you will play a vital role in delivering direct patient care, coordinating with the interdisciplinary team, and ensuring that our patients receive the best possible care experience.

**Responsibilities:**

* Provide direct patient care to hospice patients, including pain and symptom management
* Implement nursing practice following a comprehensive assessment and plan of care
* Provide accurate and timely documentation of patient services
* Assess and provide patient and family/caregiver education
* Participate in coordination of hospice services
* Ensure compliance with all state and federal legal and regulatory requirements

**Qualifications:**

* Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation
* 1 year nursing experience required, 3+ years preferred
* Experience in community health, hospice or med/surg preferred
* Knowledge of the hospice philosophy of care
* Commitment to clinical and documentation excellence
