Job Summary

MSA Home Health & Hospice, a leading provider of comprehensive home healthcare services, is seeking an experienced Full-Time Weekend Licensed Practical Nurse (LPN) to join our Hospice team in Montgomery, VA.

As a key member of our multidisciplinary team, the LPN will be responsible for delivering high-quality nursing care to our Hospice patients under the direction of a registered nurse and physician's plan of care.

Key Responsibilities:

  • Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care.
  • Adheres to policies and procedures to ensure continuity of care.
  • Collaborates with other healthcare professionals to achieve optimal patient outcomes.
  • Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
  • Participates in on-call rotation to ensure 24/7 patient care.

Requirements:

  • Successful completion of a certified licensed practical nursing program.
  • Current licensure as an LPN in the state of VA.
  • Minimum of one year of clinical experience.
  • Hospice experience strongly preferred.
  • Reliable transportation and company required auto liability insurance.
