Job Summary

We are seeking an experienced Social Services Coordinator to join our team at Liberty Healthcare and Rehabilitation Services. As a key member of our team, you will be responsible for providing exceptional care and support to our residents and their families.

Key Responsibilities
  • Develop and implement new programs and services to enhance the quality of life for our residents.
  • Conduct interviews with prospective residents and their families to assess their needs and provide guidance on our services.
  • Collaborate with healthcare professionals, social service agencies, and families to ensure seamless transitions and care coordination.
  • Provide emotional support and counseling to residents and their families, addressing their social, emotional, and spiritual needs.
  • Assist with the admission process, ensuring that all necessary documentation is completed and that residents are settled into their new environment.
  • Participate in quality assurance activities to monitor and improve the quality of care and services provided.
  • Develop and maintain relationships with community resources and organizations to provide residents with access to additional support services.
  • Provide education and training to staff on social services and care coordination best practices.
  • Perform other duties as assigned to support the overall mission and goals of Liberty Healthcare and Rehabilitation Services.
Requirements
  • High school diploma or equivalent required; one year of experience in healthcare or long-term care preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to problem-solve and think critically in a fast-paced environment.
  • Valid driver's license and reliable transportation required.
  • Ability to work well under pressure and maintain a calm and professional demeanor in challenging situations.
