Health Care Navigator Coordinator

We are looking for a Health Care Navigator Coordinator to join our team at MedPOINT Management/HCLA. In this role, you will be responsible for contacting Medi-Cal members who require health care improvement interventions and educating them on the importance of scheduling appointments with their PCP office.

Key Responsibilities:

  • Using HCLA databases, contact potential patients and provide them with information on the benefits of completing health screening tests and exams.
  • Assist members in making their appointments and ensure they have access to necessary care.
  • Regularly report on member engagement and health outcomes to HCLA leadership and health plan representatives.
  • Document outreach activity, including productivity tracking, on a weekly, monthly, and quarterly basis.

Requirements:

  • Bilingual English and Spanish required
  • High school diploma or equivalent required; Bachelor's degree preferred
  • Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook)
  • 2-3 years of experience in a managed care healthcare delivery system
  • Organized with excellent communication and writing skills
  • Excellent listening skills
  • Understanding of medical terminology
  • Compassionate and empathetic towards the needs of the patient community

Estimated Salary: $70,000 - $80,000 per year, based on experience
