**Job Title:** Comprehensive Healthcare Navigator

**Salary:** $55,000 - $65,000 per year

The Los Angeles Christian Health Centers (LACHC) is seeking a highly motivated and compassionate Comprehensive Healthcare Navigator to join our team. As a key member of our outreach team, you will be responsible for coordinating healthcare services for vulnerable populations in Los Angeles.

Key Responsibilities:

  • Care coordination: Develop and implement individualized care plans to address patients' medical, behavioral, and social needs.
  • Community outreach: Engage with community partners, attend meetings, and participate in events to promote LACHC's services and increase patient engagement.
  • Patient support: Provide emotional support, education, and guidance to patients and their families to empower them to take control of their health.
  • Collaboration: Work closely with healthcare providers, social workers, and other stakeholders to ensure seamless care transitions and optimal patient outcomes.

Requirements:

  • Bachelor's degree in social work or a related field.
  • 2+ years of experience in case management or a related field.
  • CPR certification required; bi-lingual Spanish preferred.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
