Job Summary

We are seeking a compassionate and skilled Homeless Housing Navigator Coordinator to join our team at Los Angeles Christian Health Centers (LACHC). In this role, you will provide intensive housing-focused care management services to patients experiencing homelessness and chronic homelessness under the Medi-Cal Community Supports programming.

Your primary responsibility will be to work directly with patients to complete comprehensive housing assessments, develop collaborative housing goal plans, and secure permanent housing. You will also provide intensive support for up to one year after move-in, helping patients navigate the complexities of homelessness and empowering them to achieve stable, permanent housing.
