Healthcare Navigator

We are looking for a detail-oriented and compassionate Care Management Coordinator to join our team at Gracelight Community Health. In this role, you will work closely with patients, families, and healthcare professionals to coordinate care, navigate resources, and advocate for individualized support.
Key Responsibilities- Patient Engagement and Support: Engage patients in care management programs, facilitate assessments, and provide education on disease management and health promotion.
- Communication and Collaboration: Effectively communicate with patients, families, and healthcare teams to ensure clear understanding of care plans, treatment options, and available resources.
- Compliance and Record-Keeping: Maintain accurate records, follow established protocols, and ensure compliance with regulatory requirements.
- Quality Improvement and Development: Participate in quality improvement initiatives, analyze data, and contribute to process enhancements that drive positive patient outcomes.
Estimated Salary: $35,000 - $50,000 per year
Preferred Qualifications:
- Medical Assistant Certification
- Experience with Electronic Health Records (EHRs)
