Geriatric Nurse

Bloomfield Center is seeking a skilled Licensed Practical Nurse (LPN) to join our interdisciplinary team in providing high-quality care to our geriatric and long-term care patients. As a key member of our team, you will be responsible for delivering efficient and effective nursing care, achieving positive clinical outcomes, and ensuring patient/family satisfaction.
Key Responsibilities- Deliver high-quality nursing care to patients, including evaluating their conditions, collecting data, and reporting changes.
- Contribute to nursing assessments and care planning, ensuring that patients receive individualized care.
- Supervise and monitor patient care provided by unlicensed staff, ensuring that care is delivered in a safe and effective manner.
- Collaborate with the interdisciplinary team to achieve the best patient outcomes and quality of life.
- Must be a graduate of an approved school for practical or vocational nursing.
- Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
- Experience in rehabilitative or geriatric nursing is preferred.
- Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
