Case Manager

We are seeking a skilled and compassionate Case Manager to join our team at the Goldie B Floberg Center For Children. As a Case Manager, you will play a vital role in supporting individuals with their personal goals and desired outcomes.
Key Responsibilities:
- Conduct regular home inspections to ensure the safety and happiness of individuals.
- Work with Program Supervisors to ensure coaching is completed for performance discrepancies.
- Provide regular updates to guardians on individuals' progress.
- Participate in licensing and quality inspections.
- Collaborate with stakeholders to address concerns and provide support.
- Ensure individuals have uninterrupted access to financial supports.
- Assist with initial intake and annual risk/support assessments.
- Conduct data collection compliance reviews.
- Teach human rights and sex education programs.
- Complete Supports and Risk assessments annually.
- Register individuals for school and follow up with guardians for consents.
Requirements
Must possess a high school diploma or GED.
Must be at least 21 years old.
Must possess a valid driver's license and have reliable transportation.
Must meet all of the requirements and be able to perform all of the essential job functions of the DSP position.
Must have memorized our mission and values.
Must be willing to lead by example in terms of implementing our mission, acting in ways consistent with our values.
Working Conditions
This is a full-time position that requires a staff member to work a minimum of 80 hours per two-week pay period.
The Case Manager works primarily during office hours (7:30 a.m. – 4:00 p.m.) with flex scheduling.
Operational needs will require working more than 80 hours per pay period at times and working outside of office hours at times.
The Case Manager spends a significant component of their time working hours in the field directly with individuals and Direct Support Professionals.
This time would accumulate to average a minimum of 25% of working hours.
Achieving this may involve working weekly evening hours and monthly weekend hours.
The Case Manager shares a rotating program on-call that will require assisting people served and DSPs, during and after business hours and on weekends.
