We are seeking a highly motivated and compassionate Pathway of Hope Case Manager to join our team at The Salvation Army Central Territory.
Job Summary:The Pathway of Hope Case Manager will provide direct service, community resource development, and casework consultation to ensure the full implementation of the Pathway of Hope model within the assigned Corps cluster. This role will work closely with clients, community agencies, and other stakeholders to provide holistic social service assistance and support.
Key Responsibilities:- Provide intake and assessment services as defined by the Pathway of Hope curriculum, assessing needs and verifying eligibility.
- Determine appropriate assistance grants to be awarded, verify information given, and initiate procedures for assistance.
- Work with at least 15 families over the course of one year, maintaining a caseload of 8 families at any given time, and providing interventions such as engagement of new clients, comprehensive assessments, action plans, referrals to services, and plan developments.
- Enter client data and reporting of information into the SIMS system, supporting local, national, and territorial POH program outcomes.
- Maintain case files for each client, including assessments, action plans, case notes, and graduation plans.
- Participate in POH planning and program development at assigned sites, as well as scheduled cluster meetings, conference calls, and individual consultation/supervision meetings with the Divisional Pathway of Hope Regional Coordinator.
- Participate in Divisional or Territorial educational forums and webinars to facilitate understanding of topics such as motivational interviewing, cultural competency, strength-based case management, stages of change model, smart goals, time management, advocacy, and more.
- Maintain working relationships with community agencies to provide comprehensive services for participants and stay abreast of current trends and resources available.
- A social work major is preferred, but other human service fields will be considered.
- Requires two years' experience in case management.
- Central Territory Caseworker Certification Program completion is required upon hire.
