Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine hospice needs

  • Provide health care instructions to patients and families, and assist with activities of daily living
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
  • Participate in quality assessment and performance improvement activities to ensure high-quality patient care
Requirements
  • Graduate of an accredited school of nursing
  • One year of recent acute care experience in an institutional setting, and one year of recent experience in home care or hospice preferred
  • Current licensure in good standing
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
