SUMMARY:

The Wound Care Nurse operates as a Unit Manager, tasked with delivering both leadership and specialized skin care to residents under the medical guidance and supervision of the attending physician, the Director of Nursing, or the Medical Director of the facility. This role emphasizes the treatment and management of skin disorders.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Collaborate with the Director of Nursing and/or other licensed nursing staff regarding the dermatological conditions of residents within the facility.
2. Conduct thorough examinations of residents and their medical records, distinguishing between normal and abnormal findings to determine when to refer residents for further evaluation by a physician.
3. Provide comprehensive assessment and diagnostic services to residents, utilizing techniques such as observation, inspection, and palpation.
4. Initiate requests for consultations or referrals as necessary. Respond to inquiries from residents, physicians, or nursing personnel.
5. Identify, manage, and treat specific skin conditions and lesions, including skin abrasions, foot issues like corns and calluses, pressure ulcers, and various skin infections.
6. Assist in rehabilitation activities for residents, performing range of motion exercises, aiding in ambulation, and providing emotional support while monitoring the outcomes of physical therapy.
7. Deliver preventative health care services, offering recommendations to residents regarding nutrition and the prevention of common skin disorders.
8. Prepare written and oral reports/recommendations for the attending physician, Medical Director, or Director of Nursing concerning the care and status of residents.
9. Implement and uphold established policies and procedures related to nursing care treatments, interpreting these for physicians, resident family members, and the public as appropriate.
10. Perform additional duties as assigned.

QUALIFICATIONS:
To excel in this role, candidates must demonstrate the ability to fulfill each essential duty satisfactorily. The following qualifications are essential:

- Current unencumbered state licensure as an LPN/RN.
- Wound Care Certification from an accredited institution is preferred.
- Valid CPR certification.
- A minimum of two (2) years of experience as an LPN/RN.

- Ability to move intermittently throughout the workday.

- Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a height of 5 feet and carry such weight a minimum distance of 50 feet.
