We are seeking a skilled Nurse Practitioner to join our team at Meridian Health Services. As a key member of our multi-disciplinary treatment team, you will provide professional medical care and collaborate with treating physicians to assess and monitor patients' physical and mental health status.

The ideal candidate will have a master's degree in nursing from a Nurse Practitioner program, be eligible for prescriptive authority, and possess current Registered Nurse (RN) licensure in the state of Indiana. Additionally, a Nurse Practitioner certification in Family Practice, Psychiatric/Behavioral Health, and/or Pediatrics is required.

Responsibilities will include providing direct nursing care, assessing and collecting patient/client physical examinations, making differential diagnoses, and developing and implementing treatment plans. You will also maintain timely, complete, and accurate clinical and administrative records in accordance with center policies and procedures.

Key Responsibilities:

  • Provide direct nursing care using advanced scientific knowledge, nursing theory, and skills.
  • Assess, collect, and perform patient/client physical examinations.
  • Make differential diagnoses of symptoms, functional problems, and risk behaviors.
  • Develop and implement treatment plans.
  • Maintain timely, complete, and accurate clinical and administrative records.

Experience and Education:

  • Master's degree in nursing from a Nurse Practitioner program.
  • Eligible for prescriptive authority (DEA license) under the criteria outlined by the Indiana State Board of Nursing.
  • Current Registered Nurse (RN) licensure in the state of Indiana.
  • Nurse Practitioner certification in Family Practice, Psychiatric/Behavioral Health, and/or Pediatrics.
