Job Overview

Position Summary: Under the guidance of the Director of Programs, the Senior Mobile Program Coordinator plays a vital role in supporting active and at-risk seniors in accessing essential social services, promoting their independence and well-being. This role serves seniors and adults with disabilities residing in various housing locations by providing social services, wellness initiatives, referrals, and comprehensive support.

Organizational Background: The Bernal Heights Neighborhood Center (BHNC) is a community-focused non-profit organization established to enhance the ethnic, cultural, and economic diversity of the area. Our mission is to foster community action that leads to a just and equitable society for all, particularly for individuals with low to moderate incomes. BHNC addresses community needs through:

  • Development of affordable housing across the region.
  • Provision of culturally and linguistically appropriate services to vulnerable adults, seniors, youth, and families.
  • Empowerment and leadership development for tenants, clients, and community members.

BHNC operates through five primary program areas: Housing Development, Senior Services, Youth Programs, Employment Initiatives, and Community Organizing, serving a diverse population annually.

Key Responsibilities:

  • Conduct intakes and assessments for participants in the Active Senior Program, ensuring accurate data entry using the designated systems.
  • Prepare participant documentation, including charting and monthly reports, as required by BHNC and funding entities.
  • Deliver direct short-term social services, including application assistance, translations, and information referrals.
  • Assess client needs, follow up, and document progress to ensure effective service delivery.
  • Provide administrative support during programming, including activities, field trips, and newsletters.
  • Engage with frail, at-risk seniors and low-income disabled adults to encourage program participation.
  • Coordinate meetings with the advisory board and assist in outreach efforts to increase board membership.
  • Assist in scheduling and overseeing instructors for programs.
  • Collaborate with property management to address client needs at housing sites.
  • Maintain detailed records of volunteer activities and training.
  • Recommend improvements to enhance program effectiveness.
  • Respond to public inquiries regarding volunteer opportunities.
  • Participate in BHNC meetings and events as required.
  • Perform additional duties as assigned.

Qualifications:

  • Minimum of two years of experience working with seniors and disabled adults.
  • Experience in outreach and community engagement preferred.
  • Proficiency in Microsoft Office Suite and Google Workspace.
  • Familiarity with data entry systems is a plus.
  • Ability to work collaboratively in a dynamic environment.
  • Strong customer service skills and ability to engage with diverse populations.
  • Excellent organizational and communication skills.

Requirements:

  • Bilingual in English and Spanish.
  • BA degree or equivalent experience.
  • Valid Driver's License with a clean driving record.
  • First Aid, CPR, and AED certification preferred.
