JOB DESCRIPTION

The Clinical Care Coordinator, RN, is an integral part of the multidisciplinary team dedicated to addressing the comprehensive physical, medical, psychosocial, emotional, and spiritual needs of patients and families navigating terminal illnesses and bereavement. This role is responsible for overseeing care coordination, ensuring ongoing assessment of patient and family requirements, and executing the interdisciplinary care plan.

Key Responsibilities

  • Management and Oversight for Clinical Care Coordinator, RN
  • Adherence to organizational policies and standards while practicing within the RN license scope and understanding hospice eligibility criteria
  • Reporting any requests to operate outside the licensed scope to the supervisor
  • Supervising nursing tasks assigned to Hospice Aides, Licensed Practical/Vocational Nurses, and Volunteers according to the care plan
  • Conducting initial and continuous comprehensive assessments, including physical, psychosocial, spiritual, and bereavement evaluations to alleviate end-of-life symptoms
  • Identifying and utilizing appropriate resources and referrals such as Physical Therapy, Occupational Therapy, Home Health, Dietary, Pharmacy, and Medical Equipment
  • Completing documentation promptly at the site of care
  • Educating and training patients, families, and caregivers on patient care, including pain management strategies
  • Participating in patient care meetings and weekly team discussions
  • Providing effective communication and shift reports through various methods
  • Engaging in bereavement support activities
  • Taking part in on-call rotations and providing care during evenings, weekends, and holidays as needed
  • Delivering care within the designated program coverage area
  • Collaborating with the Interdisciplinary Group (IDG) to create, coordinate, and implement individualized care plans
  • Obtaining physician orders when necessary
  • Delivering specialized nursing services requiring advanced skills
  • Evaluating the effectiveness of provided services
  • Appropriately delegating nursing tasks to Hospice Aides, Volunteers, and Licensed Practical/Vocational Nurses
  • Ensuring all factors related to patient safety are assessed before delegating tasks
  • Providing nursing care that may be delegated to other roles
  • Monitoring compliance with patient and family care activities
  • Professionally managing and coordinating the hospice care plan
  • Following infection control protocols
  • Delivering care in accordance with the patient’s individualized care plan.
  • Participating in Quality Assessment and Performance Improvement (QAPI) initiatives as assigned.
  • Engaging in Performance Improvement Projects (PIP) and gathering clinical data for quality measures
  • Fostering professional development by setting and achieving personal growth goals
  • Attending orientation and scheduled educational sessions as required
  • Participating in the orientation of new team members
  • Engaging in internal and external training activities
  • Providing exceptional customer service and maintaining a focus on patient and family care experiences
  • Acting as an advocate for patients and families
  • Reporting any patient or family complaints to the Team Manager
  • Participating in conflict resolution as applicable
  • Representing the nursing profession and the organization positively

Job Requirements

  • Reliable transportation with adequate insurance coverage.
  • Availability of a telephone (cellular or landline)

While performing the duties of the Clinical Care Coordinator, RN, the employee is regularly required to:
  • Assist in lifting and transferring patients as necessary.
  • Lift objects exceeding 20 pounds
  • Stand or walk for more than one hour per day
  • Communicate effectively

Specific vision abilities required for the Clinical Care Coordinator, RN include:
  • Close vision
  • Distance vision
  • Ability to adjust focus

The Clinical Care Coordinator, RN will be required to sit, stoop, kneel, and crouch.

Qualifications For The Clinical Care Coordinator, RN

  • Currently licensed to practice nursing in the relevant state.
  • A minimum of two years of nursing experience in hospice, home health, or community health within the last five years.
  • Equivalent experience or licensure may be considered

SUPERVISION RECEIVED

Reports directly to the Team Manager.
