The Registered Nurse Care Coordinator (RNCC) will deliver direct patient care services to hospice patients while offering essential support to their families. You will lead an interdisciplinary team (IDG) comprising Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and other support staff to enhance the quality of hospice care. Additionally, you will oversee all assigned patient clinical records.
Key Details:*This position operates Monday through Friday, with no weekend shifts.
- Must be a graduate of an accredited nursing program.
