POSITION OVERVIEW
As a Clinical Nurse Care Coordinator, you will provide direct care services to patients in hospice and offer assistance to their families. You will also lead a multidisciplinary team to enhance the quality of hospice services and manage all clinical documentation for assigned patients.
*Work schedule: Monday to Friday, 8:30 am to 5:00 pm, weekends off*
ESSENTIAL QUALIFICATIONS
- Graduate of an accredited nursing program
- At least one year of experience in oncology, long-term care, hospice, or home health
- Current state nursing license
- CPR certification
- Valid automobile insurance and driver's license
